The jurat of an affidavit is a statement set out at the end of the document which is signed by the person making the affidavit (the deponent). It includes the name of the person who the affidavit is made before, namely a solicitor or commissioner of oaths, and their full address.
Execution of the jurat is essential for the effectiveness of the document. The form is as follows (absent formatting problems using html):
Sworn by [deponent's full name] )
AT )
[Full address] )
)
) …………………………………
This [day]th day of [month] [year]
Before me
………………………………………………………
Public Notary/Commissioner of Oaths/Solicitor
The person witnessing the swearing of the document usually ascribes a stamp which sets out their name and address, rather than writing out these details.
